A Network Model of Hand Hygiene: How Good Is Good Enough to Stop the Spread of MRSA?

BACKGROUND
Simulation models have been used to investigate the impact of hand hygiene on methicillin-resistant Staphylococcus aureus (MRSA) transmission within the healthcare setting, but they have been limited by their ability to accurately model complex patient–provider interactions.
METHODSUsing a network-based modeling approach, we created a simulated neonatal intensive care unit (NICU) representing the potential for per-hour infant–infant MRSA transmission via the healthcare worker resulting in subsequent colonization. The starting prevalence of MRSA colonized infants varied from 2% to 8%. Hand hygiene ranged from 0% (none) to 100% (theoretical maximum), with an expected effectiveness of 88% inferred from literature.
RESULTSBased on empiric care provided within a 1-hour period, the mean number of infant–infant MRSA transmissible opportunities per hour was 1.3. Compared to no hand hygiene and averaged across all initial colonization states, colonization was reduced by approximately 29%, 51%, 67%, 80%, and 86% for the respective levels of hygiene: 24%, 48%, 68%, 88%, and 100%. Preterm infants had a 61% increase in MRSA colonization, and mechanically ventilated infants had a 27% increase.
CONCLUSIONSEven under optimal hygiene conditions, horizontal transmission of MRSA is possible. Additional prevention paradigms should focus on the most acute patients because they are at greatest risk.

Knowledge about the hormonal control of grain dormancy and dormancy loss is essential in wheat, because low grain dormancy at maturity is associated with the problem of pre-harvest sprouting (PHS) when cool and rainy conditions occur before harvest. Low GA (gibberellin A) hormone sensitivity and high ABA (abscisic acid) sensitivity were associated with higher wheat grain dormancy and PHS tolerance. Grains of two PHS-tolerant cultivars were very dormant at maturity, and insensitive to GA stimulation of germination. More PHS-susceptible cultivars were less sensitive to ABA inhibition of germination, and were either more GA sensitive or germinated efficiently without GA at maturity. As grain dormancy was lost through dry afterripening or cold imbibition, grains first gained GA sensitivity and then lost ABA sensitivity. These changes in GA and ABA sensitivity can serve as landmarks defining stages of dormancy loss that cannot be discerned without hormone treatment. These dormancy stages can be used to compare different cultivars, seed lots and studies. Previous work showed that wheat afterripening is associated with decreasing ABA levels in imbibing seeds. Wheat grain dormancy loss through cold imbibition also led to decreased endogenous ABA levels, suggesting that reduced ABA signalling is a general mechanism triggering dormancy loss.

A model for intraspecific competition that incorporates the effects of inter- and intra-stage survival rates, spatial distribution, and variation in growth is developed using basic probability theory. Simulation results for cannibalism in Heliothis zea (Boddie) are presented and intraspecific competition is shown to have effects on population cyclicity and on mortality.

The simulation of variability in temperature-dependent development is discussed. An algorithm for simulation of this variability is developed and validated under constant and variable temperature regimes for Anthonomus grandis, Trichoplusia ni, and Heliothis zea.

Surveillance for severe community-associated methicillin-resistant Staphylococcus aureus infection

Community-associated methicillin-resistant Staphylococcus aureus (CA-MRSA) has rapidly emerged in the USA as a cause of severe infections in previously healthy persons without traditional risk factors. We describe the epidemiology of severe CA-MRSA disease in the state of Georgia, USA and analyse the risk of death associated with three different clinical syndromes of CA-MRSA disease – pneumonia, invasive disease, and skin and soft-tissue infections (SSTIs). A total of 1670 cases of severe CA-MRSA disease were reported during 2005–2007. The case-fatality rate was 3·4%; sex and race of fatal and non-fatal cases did not differ significantly. While CA-MRSA pneumonia and invasive disease were less common than SSTIs, they were about 15 times more likely to result in death [risk ratio 16·69, 95% confidence interval (CI) 10·28–27·07 and 13·98, 95% CI 7·74–25·27, respectively]. When controlling for age and the presence of other clinical syndromes the odds of death in patients manifesting specific severe CA-MRSA syndromes was highest in those with pneumonia (odds ratio 11·34). Possible risk factors for severe CA-MRSA SSTI and pneumonia included the draining of lesions without medical assistance and an antecedent influenza-like illness.
The identification of a genetically unique piroplasma in North American river otters (Lontra canadensis)

During a routine health check of a wild-caught North American river otter (Lontra canadensis) small piroplasms were noted within erythrocytes. Analyses of the 18S ribosomal ribonucleic acid (rRNA) gene sequences determined that this was a genetically unique organism most closely related to Babesia microti-like parasites found in other small carnivores. Subsequently 39 wild-trapped North American river otters from North Carolina were tested for the presence of piroplasma deoxyribonucleic acid (DNA) via polymerase chain reaction and piroplasma DNA was detected in 82% (32/39) of these samples. Sequencing of partial 18S rRNA genes from selected cases determined that they were identical to the sentinel case. This report documents the existence of a genetically unique piroplasma in North American river otters and indicates that the prevalence of piroplasma in North Carolina otters is quite high. The pathogenic potential of this organism for otters or other species remains unknown.

We performed neutron powder diffraction on solution-derived lead zirconate titanates (PZT). Three compositions, PZT 45/55, PZT 20/80 and PbTiO3, were investigated. The materials were annealed so that the perovskite phase had just begun to grow from the precursor phase. In our materials this precursor phase is the pyrochlore rather than fluorite phase. The results show that in the pyrochlore phase, the (Ti,Zr) and the Pb are ordered in their crystallographic sites while the O are essentially disordered in both of the two usual pyrochlore anion sites.

Using electron paramagnetic resonance (EPR) we have followed the microstructural evolution with temperature of lead zirconate titanate (PZT) ceramics from the amorphous to the perovskite phase. A number of paramagnetic point defects were identified (Carbon, Pb+3, and Ti+3) while traversing the evolution of these ceramics during various heat treatments both before and after optical illumination. Perhaps the most important finding is that the Pb+3 and Ti+3 centers can only be optically created in the perovskite materials, thereby, showing that they are not associated with the amorphous or the pyrochlore phases. It is also found that EPR signals attributed to carbon radicals are present in fairly high concentrations (4 × 1017/cm3) if the solution chemistry derived PZT materials are annealed in an oxygen deficient ambient (0.1% O2) at 650°C.

Prototype ferroelectric thin film, nonvolatile memory, nondestructive readout (NDRO), semiconductor devices have been fabricated. The “1” and “0” logic states of these prototype devices are in principle determined by the modulation of the conductivity of a semiconductor film channel by the polarization state of the underlying ferroelectric thin film layer. Programmed resistance ratios of the two logic states of 5:1 are demonstrated. While the best performance to date has been achieved for devices that have a 40 nm ln2O3 film covering a 300 nm thick PZT 20/80 layer, we also develop criteria for selecting semiconductor films that will improve performance for this NDRO device design. Among the other semiconductor films that are characterized with respect to this criteria are boron doped Ge, ZnO and aluminum doped ZnO. It is demonstrated that by appropriate donor doping of ZnO films the effects of intrinsic defects are masked and that process temperatures can be extended by 300×C.

Electron paramagnetic resonance (EPR), photo-thermal deflection spectroscopy (PDS), and electrical measurements have been used to characterize as-received and UV-lluminated lead lanthanum zirconate titanate (PLZT) and PZT ceramics. Following optical illumination we observe the activation of positively charged Pb+3 and negatively charged Ti+3 ions, as well as an increased broad absorption peak around 2.6 eV. The spatial distribution of the induced absorption correlates well with the location of the absorbed UV, suggesting that photo-produced carrier pairs are trapped at Ti+4 and Pb+2 producing the observed paramagnetism. The Ti+3 EPR spectra were successfully fit using the crystal field parameters derived from the PDS spectra. Preliminary results suggest that these charged paramagnetic ions can influence the electrical properties of PZT thin films. We investigated the effects of UV light and dc bias sequences by polarization-electric field (PE) measurements and find that the coercive fields are dependent on the sign of the applied bias during UV exposure. It is reasonable to expect that Pb+3 and/or Ti+3 ions act as the charge trapping sites which cause the threshold voltage shifts in the PE loop.

Published scholarly research is asserted to be a major criteria used to judge or assess the quality of a university's faculty. This assertion has been supported in many of the academic disciplines by comparing peer ratings of graduate programs and the research productivity of the faculty of those programs. In the economics area, John J. Siegfried [ 7] found a significant relationship between peer ratings of economics departments, using the ratings of the first American Council on Education study by Cartter, and the number of available pages contributed by the department to the leading economic academic journals. A recent study by Klemkosky and Tuttle [6] found the same relationship between an academic institution's published research productivity and the quality of the institution's graduate finance programs as rated by peers, using the survey results of Brooker and Shinoda [3] to determine peer ratings. The importance of published research in turn is recognized by many academic institutions since the relative quality and quantity of published research is one of the major criteria used in making faculty promotion and tenure decisions.
